A flyleaf notes the following: "This book was made by Phyllis [indecipherable] Clay (later Kennedy) whilst an art student 1916 approx for John Tremlett"
Each part has four leaves with a color illustration by Seaghan Mac Cathmhaoil and a musical score arranged by Charlotte Milligan Fox
